adhd-md：把文档改成读得下去的样子

让模型「帮我优化一下这个文档的排版」，大概率会得到这么几样东西：几个 emoji、一堆两行小节，以及悄悄消失的两句约束条件。

做了个工具来解决这个问题，把输出的 markdown 改成更适合 ADHD 的版本。
